Enhanced Home Health Care in Rural Regions through the HÄPPI Approach

The HÄPPI project, an innovative approach to optimizing and further developing primary care in rural areas, has been gaining traction in the state of Baden-Württemberg. This concept, known as 'Primary Care Center - Interprofessional Patient Care', has been supported by the Cabinet Committee on Rural Areas during the pilot phase in ten primary care practices across the region.

The goal of the pilot phase was to identify opportunities and challenges in implementing the HÄPPI concept. The project focuses on the management of chronically and multiply ill patients, with GPs able to focus more on complex cases. A central factor for comprehensive care is the eye-level cooperation between doctors and non-medical academic health professions in the practice.

Digitalization, health, and rural areas are key areas of focus for the Cabinet Committee on Rural Areas. The HÄPPI concept improves practice organization and diagnostics through the targeted use of digital technologies. This aids in early disease detection and effective prevention and management of chronic conditions.

The successful cooperation of the Baden-Württemberg Association of GPs, the AOK Baden-Württemberg, and the state of Baden-Württemberg is crucial for securing medical care in rural areas. The HÄPPI project, along with the External: Primary Care-Centered Supply (HZV), sets standards for the future of health policy in Baden-Württemberg.

The Cabinet Committee on Rural Areas, a significant instrument of the state government for processing central, cross-departmental, and complex issues, proposes necessary measures to the ministerial council or determines them within its area of responsibility. Examples of measures determined by the committee include the awarding of model projects, the development of forward-looking concepts, the initiation of federal council initiatives, and the establishment of inter-ministerial working groups.

The Cabinet Committee on Rural Areas focuses on the state goal of promoting equal living conditions, infrastructures, and working conditions throughout the entire state. The findings from the HÄPPI pilot phase were compiled in a best practice handbook for interested primary care practices.

Dr. Jan Fedorcak, a GP in Schwäbisch Hall, is testing the HÄPPI concept as part of a statewide project. Healthcare professionals in the HÄPPI concept are more strongly involved in patient care, which allows for better management of the increasing number of chronically and multiply ill patients.
