Enhancing home healthcare services in rural locations through the HÄPPI concept
In a bid to secure medical care in rural areas and enhance the attractiveness of General Practitioner (GP) professions, a new and innovative approach called HÄPPI (Primary Care Centre - Interprofessional Patient Care) has been introduced in Baden-Württemberg.
The HÄPPI concept, which optimizes and develops primary care, encourages eye-level cooperation between doctors and non-medical academic healthcare professionals in the practice. This central factor for comprehensive care has been proven to make practice organization and diagnostics more efficient through the targeted use of digital technologies.
Dr. Jan Fedorcak, a dedicated doctor in Schwäbisch Hall, is currently testing this new GP care model within the statewide HÄPPI project. The Cabinet Committee on Rural Areas in Baden-Württemberg supported the HÄPPI project during its pilot phase in ten primary care practices.
GPs play a central role in medical care, providing long-term patient accompaniment that enables high-quality care, early disease detection, and effective prevention and management of chronic conditions. The HÄPPI concept allows GPs to focus more on complex cases and cater to the increasing number of chronically and multiply ill patients.
The successful cooperation of the Baden-Württemberg Association of GPs, the AOK Baden-Württemberg, and the state of Baden-Württemberg is crucial for ensuring medical care in rural areas. Findings from the pilot phase were summarized in a best practice handbook for interested primary care practices.
The goal of the pilot phase was to identify opportunities and challenges in implementing the HÄPPI concept. The Cabinet Committee on Rural Areas, which focuses on promoting equal living conditions, infrastructures, and working conditions throughout the country, has also identified digitalization and health as important focus areas.
The committee proposes necessary measures to the ministerial council or determines them within its area of responsibility. Examples of measures include the awarding of model projects, the development of forward-looking concepts, and the establishment of inter-ministerial working groups.
